Many patients, their families and friends have asked how they can express their appreciation to CNS for the wonderful care they or a loved one received. There are many ways of giving, now and in the future, that help make it possible to provide others with the same compassionate quality care given to you or a loved one.

Methods of Giving

All gifts are acknowledged, greatly appreciated, and are tax deductible.

• Option #1: Call Us
  Simply call the Development Department at 801-639-5434 and inform our staff how you would like to proceed. We accept cash, check, payroll deduction, monthly bank draft or a major credit card.
  
• Option #2: Make an Online Donation
  Simply fill out our Secure Online Donation Form.
  
• Option #3: FAX or Postal Mail
  Please fill out and mail or FAX one of the following gift forms:

Matching Gifts

Many businesses support the charitable goals of their employees by matching gifts. By participating in a matching gifts program offered by your or your spouse's employer, you can dramatically increase the impact of your contribution. Matching gift forms are available from your or your spouse's employer.
