Care that affirms life, and regards dying as a natural process...  Through our hospice and palliative care programs, an interdisciplinary team provides care to clients at the end of life.

Emphasis is on pain and symptom control, as well as support for the clients and families. Our Hospice Services are available 24 hours a day,  7 days a week. Hospice service provides compassionate care which allows patients to live independently in familiar surroundings and is often a less expensive alternative to hospitalization.

Susan G. Komen Foundation funds are available to end-stage breast cancer patients based on need.

Community Nursing Services Hospice Services utilize an Interdisciplinary approach to care which includes:

  • Patient and Family
  • Physician,  Nurse, and Home Health Aides
  • Social Workers
  • Physical/Speech Therapists 
  • Nutritional Specialists
  • Pastoral Counselors 
  • Pharmacists 
  • Volunteers 
Service Medicare/Medicaid Compensation 

Medicare provides a comprehensive hospice benefit to patients who:

  • Have Medicare Part A
  • When hospice services are provided by Medicare hospice-certified home health agency like Community Hospice.
  • When the patient's physician acknowledges that in the normal course of a disease process the patient is entering the last six months of life.

Patients who elect the Medicare hospice benefit are eligible for A full range of home care services including:

  • Care which focuses on pain relief, symptom control and comfort
  • Assistance with the patient's personal hygiene
  • Emotional and spiritual support for patients and families
  • Medical supplies, and equipment and related drugs
  • Instruction to help the family members to assist in the care of their loved ones
  • Respite care, support and practical assistance in the care for loved ones
  • 24 Hour on-call availability of a hospice nurse or social worker
  • Bereavement support for family members
